WOW Award Nomination Form

To insure the committee has the information they need to make an informed decision please make sure that your nominations:

INCLUDE a specific example of a WOW moment, when the nominated individual did "something notable."
INCLUDE how the "something notable" is above and beyond the call of duty.
AVOID nominations that are general in nature, without specific examples, or those that document only work that is expected as a part of the nominee’s usual job responsibilities.
