Chicago:
      Access Living of  Metropolitan Chicago is a cross-disability organization staffed and governed by people with disabilities striving to reintegrate individuals with disabilities into community life.


      Casa Central, located in Humboldt Park, West Town and Logan Square, provides a comprehensive system of service programs that are created, adapted and emphasized to meet the needs of Hispanics in the Metropolitan Chicago area.


      Chicago-Read Mental Health Center is a 230-bed psychiatric hospital that serves northern Chicago and aims to promote the highest level of functioning through its comprehensive inpatient psychiatric health care.


      El Valor is the first bilingual, bicultural rehabilitation center in the state of Illinois providing numerous services for people with disabilities in the Chicago Hispanic Community.


      Family Rescue is dedicated to alleviating domestic violence in Chicago and providing shelter and support services to women and children who are victims of domestic violence.


      Levy Center, a Senior Center operated by the City of Chicago's Department on Aging, offers recreational and informational programs to older Chicagoans.


      Mile Square Health Center is a comprehensive community-based health care facility located in an impoverished neighborhood known as the "Near West Side."
