Scanning Electron Microscopic Views of White Spot lesion.
Many irregular openings may be observed on the white spot enamel surface.
Islands of enamel may be noted that have been demineralized and remineralized. The enamel surface openings can extend into cavernous, demineralized sub-surface areas.
Note that the sub-surface immediately below the enamel surface is more intact (less demineralized and more insoluble) than the demineralized zone farther below. This is due to accumulation of F-apatite near the surface.
Healing of this surface involves greater remineralization than demineralization.
