What to Bring to Your Appointment

Please make sure to bring these things to your appointment:

• Photo Identification
• HMO Referral (if needed)
• Insurance card(s)
• Names and dosages of any current medications
• Glasses/Contact Lenses

New Patient Pre-Registration

All new patients must be registered in the UIC Medical Center system before being seen in any of our clinics. To expedite your visit, you can pre-register by calling Patient Registration at (312) 996-1682.
