To refer a patient to one of our clinics, you have several options:

  2. Make an ONLINE PATIENT REFERRAL and an Appointment Coordinator will call the patient directly to schedule an appointment.

  3. FAX YOUR REFERRAL to the Clinic Fax Number listed above and an Appointment Coordinator will call the patient directly to schedule an appointment.

 

NOTE: Managed care patients may also require an "insurance-approved" referral.
