Please note: Other forms GME has sent you, including Lab Coat Order Form, Veterans Administration (VA) Appointment Form, and Resident Agreement should be returned to the GME Office BEFORE Orientation.
Please bring the items on the following list with you to Orientation. Because the GME Office cannot begin the process of putting you on payroll without these items, failure to turn them in at Orientation may further delay your first paycheck.
1. All completed Payroll papers mailed to you by GME, which may include:
I-9 form
Acknowledgment of Mandated Reporter Status
Drug-Free Workplace Statement
Non-US citizens should also bring:
W-4
IL-W-4
Employee Withholding Allowance Certificate
Foreign National Tax Information Form
Request for Permanent Revalidation of ECFMG Certificate
2. The necessary identification and proof of eligibility to work in the US
The list of acceptable documents is listed on the back of the I-9 form or available from the USCIS - US Citizenship & Immigration Services (formerly INS & BCIS) website.
3. Any previous University of Illinois ID card
If you have an ID card from any University of Illinois campus, you will need to return it in order to get your new I-card.  The ID Office charges a substantial fee for failure to return a previous ID card.
4. A copy of your medical school diploma if you are a 2006 graduate and haven't supplied one yet to your program or the GME Office.
5. Your NESSIE ID and password (sent to you by your GME Program Contact)
You will need to make your benefits selections on NESSIE within 10 days of your contract start date.
6. Your checkbook if you will be purchasing parking at the University.
