Requesting a Duplicate Certificate
To request a duplicate, replacement or revised copy of your UIC residency and/or fellowship training certificate(s), please send the following items to the GME Office:
1. A Notarized Business Letter Requesting the Certificate(s)
The letter must be notarized and must include the following information:
   Your full name at the time your residency or fellowship training at UIC ended,
   The name to be used on the reprinted certificate (if different),
   The name of the training program,
   The start and end dates of your training, and
   Your current contact information (address, telephone number, email address).
Please note: If you are requesting a certificate for more than one UIC residency and/or fellowship training program, you may include the program names and training dates in a single letter.  Separate requests are not required.
Also note: If you have common first and last names, please feel free to also provide your UIN (University Identification Number) or the last four digits of your SSN.
2. Proof of Name Change (If Applicable).
If you have changed your name and are requesting a certificate printed in your new name, you must also submit a copy of the appropriate legal document and a copy of your replacement Social Security card in your new name.  Acceptable name change documents are:  Marriage Certificate, Divorce Decree, Affidavit or Court Order.
3. Personal Check or Money Order payable to:  
Graduate Medical Education/Univ of IL at Chicago.
The cost for each certificate requested is $25.00
($50 for two; $75 for three . . .).

Please allow six to eight weeks for the printing and delivery of your duplicate certificate(s).
