As a University of Illinois Medical Center at Chicago (UIMCC) resident, you are a mandated reporter of child abuse and neglect, and consequently are required to know the Illinois Abused and Neglected Child Reporting Act [325 ILCS 5/4]. A summary copy of the law is available for review in the Graduate Medical Education Office and available through the Illinois General Assembly Compiled Statutes website. |

UIMCC Child Abuse or Neglect Reporting Procedure

 |
1.
|
Gather documented information to evaluate whether there is a case of suspected child abuse or neglect. |
 |
 |
 |
 |
 |
2.
|
Consult with the Department of Health Social Work and any appropriate medical services regarding suspected child abuse or neglect in order to complete evaluation prior to reporting. |
 |
 |
 |
 |
 |
3.
|
In a tactful, non-accusatory manner, inform parent(s) or other responsible adult why a report is being submitted. |

 |
 |
 |
4.
|
Call the 24-hour DCFS State Central Registry (Child Abuse Hotline: 1-800-25-ABUSE.) Following collaboration with the medical social worker, the staff member who has identified a suspected case of child abuse or neglect should make this call. If known, the following information is provided: |

a.
|
Name and address of child and parent or other persons having custody; |
 |
 |
b.
|
Child's age; |
 |
 |
c.
|
Nature of child's condition, including evidence of previous injuries (when appropriate, physician should record visually evident marks on CANTS Form 418-0302, http://www.state.il.us/dcfs/cants4.doc.) |
 |
 |
d.
|
Other information which might be helpful in establishing cause of child abuse and neglect (CA/N) and who may have perpetrated it. This information must be confirmed in writing after the initial telephone report; and |
 |
 |
e.
|
Indicate whether local law enforcement agencies are aware of suspected CA/N if appropriate. |

5.
|
Give the completed CANTS form and any supporting documentation to the medical social worker on service. The social worker then reviews and submits completed form to the Coordinator of the Child Protection Team (6-0293.) |
