Complete Residency File:
File requirements include but are not limited to:
   Application
   Updated CV
Please note: Your CV must be updated after you begin your program to include activities between graduation from medical or dental school and the first day in your residency. You will not be able to clear at the end of  your program if an updated CV is not on file in the GME Office.
   Personal Statement
   USMLE Board Scores (or equivalent)
   ECFMG: Certificate / Status Report
   Diploma
Please note: You must bring a copy of your diploma to GME New Resident Orientation or provide one to your GME Program Contact as soon thereafter as possible if you have not yet received it.
   Dean's Letter
   Medical or Dental School Transcripts
   Three (3) Letters of Recommendation
  
Licenses:
Your program office will send you the appropriate Illinois medical license application with instructions included. Fill out the application immediately and return it to the GME Office along with the application payment made out to the IDFPR (Illinois Department of Financial and Professional Regulation). The GME Office address is as follows:

Human Resources:
The GME Office will send you a NESSIE login ID, NESSIE password and instructions for accessing NESSIE, the Net-driven Employee Self-Service and Information Environment, the University of Illinois web-based program for Human Resources, Benefits and University Payroll offices. DO NOT DELAY ACCESSING NESSIE AND COMPLETING ALL REQUIRED SECTIONS.
   YOU CANNOT BEGIN WORK OR BE PAID IF YOU FAIL TO COMPLETE THE NESSIE FORMS.
   YOU CANNOT BE PAID ON TIME IF YOU FAIL TO COMPLETE THE NESSIE FORMS IN A TIMELY MANNER.
  
Health Screenings:
It is the UIMCC policy in accordance with OSHA and JACHO requirements, that all health care workers must successfully complete this MANDATORY evaluation prior to working in the Medical Center. You will not be placed on the payroll and will not be allowed to work if this is not completed. I-9 Forms:
The I-9 form proves that a person may legally work in the US.  Human Resources will not put anyone (including US citizens) on payroll without a completed I-9 form.  You must bring the required forms of identification to GME New Resident Orientation.  The allowable forms of identification are listed on the back of the form, which will be mailed to you prior to Orientation.  The most commonly used forms of identification are US passport, Social Security card, drivers license and birth certificate.  According to federal law, the I-9 cannot be backdated more than three days.  Therefore, if you do not bring the proper ID to Orientation, you must go to the GME Office within three days of the start of your Resident Agreement.
  
Visas and Work Clearance Forms:
Every non-US citizen needs Work Clearance and I-9 forms filled out by the Office of International Services (OIS) to begin working.  You cannot work without Work Clearance.  Work Clearance and I-9  forms cannot be backdated more than three days.  Therefore, even if you have a valid visa, but haven't shown OIS the proper identification, you cannot be paid prior to the date on the Work Clearance.  Furthermore, this date becomes the start date of the residency.
  
Social Security Numbers:
If you do not have a Social Security number, you cannot be put on payroll, have Hospital intranet PowerChart access, or be issued a Nessie ID to sign up for benefits or fill out new hire forms.  Foreign residents must visit a Social Security office to request a number as soon as they reach Chicago.  For more information about Social Security numbers or offices, see www.ssa.gov.
