Welcome to your University of Illinois at Chicago residency or fellowship program. As a foreign medical graduate, you have been sent a standard ECFMG certificate with examination dates subject to expiration: The English exam date and the Clinical Skills Assessment exam date. Because you are entering one of UICs residency or fellowship programs, you will be eligible to apply for permanent revalidation of your ECFMG certificate. |

For your convenience, a copy of the Request For Permanent Validation Of Standard ECFMG Certificate form has been sent to you. Please follow all instructions on the reverse side of the form and return it to GME at orientation. |

The form will be sent to ECFMG when you have started your residency or fellowship at UIC. The processing time is 4-6 weeks. Once GME receives the valid indefinite sticker, you will be asked to bring your original ECFMG certificate to the GME office so that the sticker can be applied. |

For residents who have already received a permanent revalidation of their ECFMG certificate, please submit a copy to GME at orientation. If you have any questions, please call Denise Hicks at 312-996-2936 or send her email. |

NOTE: Section I of the form has to be completed & signed by the applicant. However, GME has inserted its office address as the mailing address. This has been done to guarantee delivery of the sticker. In the past, because residents change their addresses, stickers have been lost in the mail. We hope to reduce the occurrence of lost stickers by having them mailed to the GME Office. |
