How to...    Under Revision for 2006 •   Get your UIC Medical Center Lab Coats, 312-996-5858 1. Fill out the Lab Coat Order Form that was sent to you in April 2004. a. If you aren't sure of the size, you may try on sample coats in the GME Office. b. To obtain another form, contact the GME Office, 312-996-2933. 2. Return the form to: Graduate Medical Education  M/C 675 University of Illinois at Chicago 820 South Wood Street Chicago, IL 60612-7311 Or fax it to GME at:    312-996-3050. 3. Pick up coats at New Resident Orientation in June 2004. •   Get a UIC Photo ID (I-card), 312-413-5944 1. To obtain a UIC I-card, you must bring a state ID, driver's license or passport to the I-card Office. The state ID or driver's license does not need to be from Illinois. 2. The I-card Office is located in room 241 CIU (Chicago Illini Union), 828 South Wolcott Avenue. 3. Please note: The I-card Office is open only from 9:00 am to 4:30 pm, weekdays, and is closed daily from 1:00 pm - 2:00 pm for lunch. You will not have an opportunity during GME Orientation to get your I-card. The University charges a fee for previously issued, unreturned University I-cards. •   Get Parking at UIC, 312-413-5850 1. Please note:  You must have an I-card in order to apply for UIC campus parking. 2. To obtain campus parking: a. Fill out the Parking application. b. Bring your I-card, the filled out application and a check to the Parking Office. You may elect to have the parking fees deducted directly from your paychecks. 3. The Parking Office is located in 217 SRH (Student Residence Hall), 828 South Wolcott Avenue. The Parking Office is open from 8:00 am to 5:00 pm, weekdays.
