Change your address after you relocate: After leaving UIC, to receive your statement(s) of wages in January 2007, fax a letter with your new, permanent address to the UIC Payroll Office at 312-996-6542. A written letter is required. Telephoned requests will not be considered by the Payroll Office. |

Clearance Form: Your Clearance Form will be mailed. Contact GME at 312-996-2933, if you need a replacement. In order to receive a certificate of completion1 and verification of your residency or fellowship, you must complete the GME Clearance Form. |

If you served at least 12 months but did not complete your program, you will receive a certificate of time served. |

Copy of current Illinois Medical License: Bring a copy of your current Illinois Medical License to the GME Office. This is a Clearance requirement. Until it is on file, you will not receive a certificate of completion and the GME Office will not verify your residency or fellowship. |

Online Core Curriculum Modules:2

a. |
If you are completing a residency program this academic year, you must have completed four online Core Curriculum modules per year, or a total of ten modules for a program of three or more years. |
 |

b. |
If you are completing a subspecialty or fellowship this year, you must have completed four Core Curriculum modules per year unless you have previously completed them at UIC or can produce documentation that you completed a similar curriculum at another institution. |

 |
Core Curriculum completion is an ACGME and GMEC Clearance requirement. You will not receive a certificate of completion and the GME Office will not verify your residency or fellowship until you have completed the required number of modules. |

A few programs provide the Core Curriculum topics in-house. Your program director will inform you if your program uses an annually-approved, alternative to the Online Core Curriculum. |

Rotation Evaluations: A rotation evaluation is required for every rotation throughout your residency or fellowship program; this is a Clearance requirement. You will not receive a certificate of completion and the GME Office will not verify your residency or fellowship until you have completed the required number of evaluations.3

Beginning in summer 2007, rotation evaluations for Duty Hours and Institutional issues will be completed through the New Innovations Residency Management Suite. If you need to clear before the evaluations are available through New Innovations, call the GME Office at 312-996-2933 to request completion on paper. |

SURS Contributions: If you have questions about reclaiming the money you may have paid into SURS, contact SURS directly. The SURS address, location, telephone and fax numbers are available through the SURS website. |

Disability Insurance Conversion Offer: Your long-term disability insurance policy provided by the Medical Center during your residency may be continued on an individual basis with unisex pricing and 30% discounted premiums exclusively available through Lumsden & Associates, the insurance brokerage firm selected by the University of Illinois for institutional disability policies at the College of Medicine. No medical exam is required. The enrollment period is open through June 30, 2007. Address questions to David Lumsden's office at 1-800-789-6315. |

J-1 Visa Holders

Notification of Termination as an ECFMG Exchange Visitor in the US: J-1 visa holders also must complete the form "Notification of Termination as an ECFMG Exchange Visitor in the United States" if leaving before the end of your program. This form is available from the GME Office or from the ECFMG website. It is the responsibility of the J-1 visa holder to alert GME when they no longer need sponsorship. This form should be completed only if the exchange visitor no longer requires ECFMG sponsorship in the United States. |

Foreign Medical Graduates

Residents Leaving after PG1 Year

Teaching Skills Workshops: Three workshops are required including the one presented at Orientation in June. Completion of the workshops is part of the Core Curriculum requirements. You will not receive a certificate of time served and the GME Office will not verify your PG1 residency year until you have completed the required number of workshops. |

Residents Resigning Prior to End of Current Contract

Resigning from your program prior to the end of your current contract: You must submit a resignation letter to your program director who must accept the resignation. Additionally, you must complete the GME Clearance form. Please see the GME Policy and Procedure Manual, section 21.3, Resident Resignation on the GME website regarding the conditions of resident resignation from programs. If you are completing less than one year in your program, you will not receive a residency certificate or verification of time served from the GME Office . |
