You may take Hospital Information Systems Electronic Medical Record (EMR) training without an NPI number. However,  in order to receive your EMR access code, you must have provided your NPI number to the GME Office. GME will forward your NPI number to Patient Accounts so you can receive your EMR access code.

The Administrative Simplification provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) mandates the adoption of standard unique identifiers for health care providers. To help you meet this requirement, the University of Illinois Medical Center at Chicago (UIMCC) has provided the enclosed instructions and submission form.

Please note that you may request a paper application or apply online through the National Plan and Provider Enumeration System (NPPES) website:
https://nppes.cms.hhs.gov/NPPES/Welcome.do
2) “National Provider Identifier (NPI) Submission Form”   (1 page)
When you receive your NPI number from NPPES, PRINT your Name and NPI Number in the blanks on the form.
Please complete the submission form as soon as you receive your NPI number and return it to the GME Office at your earliest convenience. We will forward your form to the UIMCC Patient Accounts office on your behalf.
  
National Provider Identifier (NPI) Enrollment Instructions for Resident Physicians
Steps to the process:
1. Apply for an NPI either online or by paper application. [NOTE: If you do not have a U.S. social security number, you must apply ON PAPER. See below for the phone number to request a paper application.]*
2. Return the NPI "Submission Form" and a copy of the letter [or email] you will receive from NPPES with your other documents.
3. Retain a copy of your NPI information and letter from NPPES.
4. Retain your login ID and password that you establish during the online enrollment process.
*You can request a paper application by contacting the National Plan & Provider Enumeration System (NPPES) at  1-800-465-3203  (TTY 1-800-692-2326)

The online enrollment process takes 10 to 15 minutes to complete.  For online enrollment, please follow these steps:
1. Click on the link “National Provider Identifier (NPI)” for Health Care Provider
2. Click on the link "Apply Online for an NPI"
3. Read through the instructions and "Begin Application Form"
[NOTE: As a "Student in an Organized Health Care/Education/Training Program - Taxonomy 390200000X" you DO NOT need a medical license to apply for an NPI]
4. Choose an NPI User ID (make this something that you would remember, as it can not be changed and will be needed throughout your career as a Health Care Provider)
5. Choose an NPI Password (password must be 6-12 characters long, contain at least one letter, one number, no special characters, and not be the same as the user ID)
6. Select Secret Question (this will assist you in the even you forget your password and need it reset in the future)
You have now started the NPI Application Process
NOTE: BE SURE TO USE THE "Next" & "Previous" KEYS LOCATED AT THE BOTTOM OF THE PAGE; DO NOT USE THE Internet Server's [your Browser's] Back & Forward keys.
1. Be sure to fill in all fields marked with an asterisk (*)
2. Be sure to also fill in personal information as it is legally recognized (for example, please list other names you have been known as, such as Maiden name)
3. Note, unless you have a private practice as your primary practice location, you are NOT a “Sole Proprietor”
4. “Domestic Mailing Address Information” à You can select any address you prefer to have NPI information sent to. You can use your home or office mailing address.
5. “Domestic Practice Location Information” à This should be your Primary Practice Location. You need not to include other Practice Locations

6. “Add Identifier” à If you have knowledge of any legacy/identifiers (such as HFS Provider Number or UPIN) please include these.  Generally, this will not apply and you can leave this blank.
7. “Add Taxonomy” à As a resident [or fellow], you must identify yourself as 390200000X:  Student in an Organized Health Care/ Education/ Training Program.
8. “Contact Person Name” à You can choose to make yourself the Contact Person, or you can choose someone else.  This person will be able to call NPPES to make minor changes and will receive information regarding your NPI.
9. IMPORTANT*** Contact Person Email: This is the email address where you will receive your NPI Confirmation Letter -  KEEP THIS CONFIRMATION LETTER FOR FUTURE REFERENCE
10. You are finished!  Make sure to mark the [ ] on the Certification Statement and Submit.
11. You will be given a Tracking Number and should receive a confirmation email from NPPES with your NPI. If you do not receive this confirmation email within 15 days, call NPPES @ 1-800-465-3203. An NPI Specialist will be able to assist you.  Generally, we have found that NPI confirmation letters arrive within 10 to 15 minutes of your successful application submission.

National Provider Identifier (NPI) Submission Form for Resident Physicians
As a part of the enrollment process for the University of Illinois at Chicago, we are requesting that you obtain a National Provider Identifier (NPI).  Please reference the enclosed instructions for information on how to apply for an NPI.
After you have applied and obtained your NPI, please complete this form and return it to us.   You must attach a copy of the letter that you will receive from the National Plan & Provider Enumeration System (NPPES).   If you apply for this using the internet, the entire process is electronic.
