The Form |

The GME Office will accept only those application forms approved by the GMEC. These include the following: |

NRMP form for medical students |
 |
 |
2. |
COM form for all advanced positions |
 |
 |
3. |
Other forms providing they are official applications for the specialty (for instance, in the case of a specialty match requiring its own application) and the form includes all items requested on the UIC application. This includes ERAS applications. |

B. |
Distribution of applications |

 |
Program directors may send applications to candidates at their discretion, within the bounds of University anti-discrimination policies. |

C. |
Terms of Residency |

 |
 |
 |
 |
Applicants for GME programs must be informed in writing of the terms and conditions of residency and benefits, including financial support, vacations, professional leave, parental leave, sick leave, professional liability insurance, hospital and health insurance, disability insurance, and other insurance benefits for residents and their families, and the conditions under which living quarters, meals and laundry or their equivalents are to be provided. This information is contained in Exhibit A of the Resident Agreement and is available through the GME web page. |

Request for Resident Agreement |

A. |
Requests for Resident Agreements must meet the following conditions before the candidate will be offered a Resident Agreement: |
 |
 |
1. |
The GME Office receives a written request which identifies the resident, the year of training, and is signed by the program director. |
 |
 |
2. |
All applicable match rules are followed. |
 |
 |
3. |
A budgeted vacancy exists or will exist by the starting date. |
 |
 |
4. |
The request is accompanied by a file containing the applicant's original application and originals of all documentation specified in the form of application used. |
 |
 |
5. |
The candidate is eligible for Illinois medical licensure and, if not a U.S. citizen, is eligible for a visa, in the judgment of the Director of the Office of Graduate Medical Education. |
 |
 |
6. |
The candidate will have passed all parts of USMLE Step 2 (or its approved equivalent) by the starting date. |
 |
 |
7. |
The candidate must have completed the "Criminal Background and Government Exclusive List Release From" and "University of Illinois Conditional Hire Acknowledgment." |
 |
 |
8. |
In the case of a transferring resident, the program director has obtained primary source written or electronic verification of previous educational experiences and a summative competency-based performance evaluation of the transferring resident. |
 |
 |
The GME Office will notify the program director immediately if the request or documentation is not complete. |
 |
 |
 |
 |
 |
B. |
The program director must provide assurance to the GME Office that the above-mentioned conditions have been met before making an offer, verbal or otherwise, to the candidate(s). Nothing in this policy should be construed to limit the program director's choice in selecting candidates for residency positions, provided the conditions in (A) above are met. |

C. |
When a resident who has trained in one of the University's residency programs is being considered for training in another University program, the program directors must provide assurance to the GME Office that written and verbal communication has taken place between the two program directors concerning the quality of the resident's performance in the first program, as noted in Section 4.3(A) 8. above. This communication must occur before the resident is offered a position in the second program. |

The Director of the Office of Graduate Medical Education is Institutional Contact for all matches. |

The GME office will pay all match fees, if the applications and documents are submitted through the office. |

Program directors will adhere to all applicable NRMP and specialty rules as published. |
