22.1 Form
A. Certificate forms will be designed by the Office of GME to conform with University identification policy
B. Certificates for residents who complete a  program will indicate such on the certificate;               residents who complete at least one full year of training but leave  prior to completion of their training will receive a certificate that states they participated in the program.
C. Definition of program completion will generally signify that the resident has fulfilled the training requirements for the specialty board.  For fellowship programs where no board              requirements are available, the program director will determine whether a fellow has completed the program for certificate purposes.
  
22.2 Verification Letters
A. Where a resident has not completed a full year of training, the GME Office will issue a letter  certifying completion of rotations. The content of the letter will be restricted to confirming dates of training and that the resident  performed in a satisfactory manner.  The letter will be signed by the Residency Program Director.
B. Requests received by the GME Office for additional information regarding resident performance, or verification requests for residents who have experienced disciplinary problems, will be referred to the appropriate Program Director for completion.
  
22.3 Clearance
A. Certificates and letters will be issued to residents only if and when they complete all College of Medicine and affiliated hospital clearance  requirements.
  
22.4 Sign Out Sheet
A. Each resident must submit a completed GME Clearance Form to the GME Office at the time they are finishing their program or are leaving.  The GME Office will not release a training certificate or verify a residency until the resident returns the completed Clearance Form.
B. The GME Clearance Form will be approved by the GMEC.
C. Institutional clearance
1. The GME Office will require individual affiliated institution clearance for each departing resident, upon request of that institution.
2. The requesting institution will specify the nature of the clearance required.
