Physician referrals

All Patient Referrals can be obtained by calling 312-996-4942 between 8:00am and 5:00pm.

Toll free number 1-888-IL-HEART is available 24 hours a day, 7 days a week.

• Patients can be seen Tuesdays and Thursdays at the Outpatient Care Center at the University of Illinois from 1:00pm to 3:00pm
• Louis Weiss Hospital Hours are Tuesday from 9:00 am to 11:00am
• Mount Sinai Patients can be seen at Kling Outpatient Building on Wednesdays from 3:00 pm to 5:00pm.
