Bariatric surgery questions and answers

I am interested in learning more about Bariatric (Weight Loss) surgery, where do I start?
For new patients, please call 312-996-1682 to register in our system.  Registration staff will obtain necessary information and schedule you for an information session conducted by one of our 4 surgeons. 
These information sessions are held on Monday evenings at 5pm in the Outpatient Care Center, 1801 W. Taylor 3rd Floor, Clinic 3F. Sessions are approximately 2 hours in length.  Please bring your insurance card with you to information session.  We will collect needed information and our financial counselor will verify benefits.
Following the information session, you will be scheduled for an initial consultation with your chosen surgeon.

What should I bring to the initial consultation?
Insurance carriers require Medically Supervised Weight Loss documentation.
Please bring documentation of previous attempts at weight loss with you.

Psychological Testing Form
Referring physician’s name and phone number
Present medication summary
Your insurance card
Picture ID
Any questions that you would like to address with the surgeon.

It is suggested that a family member or trusted friend accompany you so that an additional person hears what the specialist recommends.
For the following visits you may need to bring only the insurance card and your picture ID. After 60 days from the surgery you will need a new referral.

What are the insurance plans accepted?
We accept many insurance plans here at the University of Illinois at Chicago.  Please be aware that Illinois Deparment of public Aid (MEDICAID) does not cover Laparoscopic Adjustable Gastric Band .
Please, be aware that insurance requirements are different based on carrier.

If I do not have insurance coverage, how much would the Lap Band procedure cost?
The following fees include the pre-op visit, surgery and hospitalization and 60 days post-op clinic visits. They do not include any pre-operative testing, lap band adjustments or further fees incurred due to complications.
These fees may change periodically and we update this information monthly.
