Reporting Non-urgent
Building Issues:
If you happen to notice a building issue in CUPPA Hall, Art & Design Hall
or one of your campus classrooms, please use the following protocol to report
the issue:
If the issue is within your unit’s suite in CUPPAH/ADH:
1. Report it to YOUR UNIT’S FMWeb contact (see
below), who will submit a request to Facilities Management
2. If your FMWeb contact is not available, report it
to a Dean’s Office contact (see below)
If the issue is in a common area like the first-floor lounge,
stairwells, elevator, classrooms or is elsewhere on campus:
1. Report it to a Dean’s Office contact
2. If your unit's service request is not completed in a timely fashion, contact
the Facilities Management Service Desk at 6-7511 and provide the service
request number
If your unit contacts Servdesk and feels that follow-up
is needed (to clarify the request or if no service is provided within a
reasonable amount of time):
1. Contact the trade shop involved in the service request (see below)
2. If further assistance is required after contacting the trade, contact the
Dean's Office FMWeb contact (see below)
Reporting URGENT Building Issues:
Urgent building issues include but
are not limited to heating and cooling issues, major leaks, power
failures. In the case of an urgent issue:
1. CALL the service request in
IMMEDIATELY to Servdesk at 6-7511 from any campus
phone
2. Inform the Dean's Office of the issue immediately after calling in the
service request
3. If it is warranted, notify the UIC Police at 5-5555 (312-355-5555)
from any campus phone
Reporting
Environmental Health and Safety issues:
1. Email or call
the Environmental Health and Safety Office immediately at:

2. Copy email requests to schorsch@uic.edu,
or inform the Dean's Office immediately after calling in the report

Further Information
We all play an important part in
maintaining and increasing the physical quality of life in our college. You can
help by reporting building maintenance and safety problems. If CUPPA staff and students
happen to notice in CUPPA Hall, Art and Design Hall or in a campus classroom
that a light bulb is out, a faucet leaking, a classroom or washroom needing a
clean-up, the washroom out of towels - anything that should concern maintenance
staff and building trades - please IMMEDIATELY inform your unit's FMWeb contact or the CUPPA Dean's Office. In case of urgent
issues, call in a report to Facilities Management Service Desk at 6-7511 from
any campus phone.
It is especially important in the
case of recurrent problems such as not enough or too much heat to report the
problems through the above system EVERY DAY, referencing back to the service
request number of your earlier unresolved report. Supervisors at Facilities
Management view these reports and will respond to unresolved problems.
If you
notice a problem early in the day, please report it via FMWeb
immediately. Otherwise, we may have to wait for the Facilities afternoon shift
to arrive, or wait until the next day. Please also do not suffer in silence if
you are uncomfortable with building conditions. We appreciate if you report
problems each time they are observed. Everyone in CUPPA is empowered to
report building problems, and staff are expected to do
so. Staff especially are asked to use the FMWeb system per the instructions above. CUPPA reviews the
aging of FMWeb complaints on a regular basis with FM
supervisors.
Of course, if you are faced with an emergency such as flood, power failure, or
any danger to life and limb, please call the UIC Police immediately at 5-5555
from any UIC campus phone.
For most routine
maintenance items, this method above works fine. Facilities Management has
taken a number of important steps to improve response time on maintenance
matters. If there is an urgent problem, such as the freight elevator being out,
a power outage, etc., you should call 6-7511and speak with someone at the
Service Desk. Please also let Albert Schorsch know at
6-2177, or at the emergency number listed on his voice mail. (If you are not
within the UIC College of Urban Planning and Public Affairs, please do not call
or page me unless the problem pertains to the floors of the 400-412
I urge you not to let a
problem go unreported, especially one that affects quality of life or public
safety. Facilities Management staff are eager to solve problems--but first we
have to let them know what the problems are! Also, if you wait to report
problems, we may not see any response until the late afternoon, or the following
day.
If a reasonable amount of
time passes after your complaint, and there is no effective action, please send
a copy of your report number to schorsch@uic.edu. The Dean's office will then
make a follow-up call, and let you know the result.
Similarly, please don't
hesitate to call the UIC police at 5-5555 (312-355-5555) if trespassers are
seen within UIC buildings, especially if panhandling or using washrooms
inappropriately. This request is for the safety of all. Thank you for your
