Program Description

Sponsored by the Chicago Department of Public Health and funded by the Centers for Disease Control and Prevention, the REACH 2010/Lawndale Health Promotion Project (LHPP) is a coalition of North and South Lawndale organizations, businesses, health service providers and residents that work together to promote healthy lifestyles in their communities. LHPP's mission is to eliminate health disparities by reducing and preventing illnesses linked to type 2 diabetes and cardiovascular related diseases among African American and Hispanics. The Division of Community Health partners with the Chicago Department of Public Health, University of Chicago and Loyola University to oversee evaluation activities for the REACH 2010 project. Activities include: Community Peer Educator Training, Health Care Provider Education, Community Coalition Development and Assessment, Community assessment, Community resident risk and knowledge assessment, University/Community Evaluation Committee.
