Please complete the following form to request a new Blackboard course site or to request a re-use/copy of a previous course site. Requests should be submitted at least two (2) weeks before the site is needed to guarantee timely setup.

Instructors using Blackboard for the first time need to attend or schedule an introductory faculty training session (see below).

If there are any questions or problems, please email the ITL Blackboard support staff at blackboard@uic.edu.

Note: Extra boxes are for additional call numbers (if needed). For lecture/discussion combinations, just list the lecture.

Comments:

In particular, indicate here if you are supplying multiple call numbers to get multiple independent sites, otherwise they will all be added to the same site.
End-of-term Policy: *

Semester-based and temporary course sites will be available until the end date you specified above, normally one week after finals. Then they are hidden from students, but remain on your list (you can override site availability and duration from the control panel, under Settings). Older sites will then be sorted towards the bottom of your course list by prepending a "z" to their name.
One year afterwards, the site will be taken offline and thus become inaccessible even to you. Nothing will be deleted from it, however.
Its materials can be re-used for future terms with new student info upon your request (via this form).

This does not apply to ongoing course sites.
       I understand.

For first-time Blackboard instructors only...

First-time Blackboard instructors need to schedule an introductory Blackboard faculty training session with the ITL, which typically lasts 2 hours.

This is to help you be more productive with Blackboard and to avoid a flood of questions later on. If you previously had such an introduction, you do not need it again. However, you are always welcome to request additional training via the ITL home page.

Please specify both a preferred and an alternate date and time for your training session:

Prefered Date and Time:
   Date (Use mm/dd/yy)
   From To

Alternate Date and Time:
   Date (Use mm/dd/yy)
   From To
Location: *    ITL West    ITL East

Note: The ITL-West is on the west campus at 1940 West Taylor, in room 181 BGRC. The ITL-East is on the east campus in room LIB 1-310, on the first floor of the Main Library, south wing, facing SEO.
