Request form for a new Mallard site

For faculty/staff only!

Please fill out the following form to request a Mallard class site or to request re-use (=copy) of a previous course. All requests must be submitted at least 2 weeks prior to the beginning of the semester. We will load your initial student roster, but you will have to add late registrants yourself, via the Mallard Roster Manager. Each term, you need to request the site again!

Note: Mallard uses bluestem-authentication, so only users with active accounts on tigger, icarus, or mailserv can use the system. Please make sure your students create their icarus-accounts right away, on the web or in the ACCC public labs.

Note: everybody using our online services needs to have a UIC NetID. For first-time Mallard developers:

You need to come to the ITL for a 90-120 minute Mallard introduction. This is to introduce you to the basic steps of developing quizzes in Mallard, and to avoid a flood of questions later on. If you previously had such an introduction, you do not need it again. However, you are always welcome to request additional training via the ITL homepage.

Please specify two dates and time slots that you are available (if other faculty/staff/TAs are going to work with you on this board, they definitely should come together with you). You need to give us at least 48 hours advance notice for this, so we can schedule the training and notify you. Note that the ITL is on west campus.
