Checkout Procedures
In order to checkout equipment from the SML you must be a current UIC student. Please make sure to review the following information to ensure you are able to checkout equipment without any issues.
Checkout Equipment Procedures
- You must bring a valid UIC i-Card ID (with picture ID)
- You must be listed in the UIC Phonebook
- It is necessary to be listed in the UIC Phonebook to verify your student status
- All equipment that is checked out will be entered into the SML database
- You are required to sign the print-out version of the checkout form
- Review all equipment listed on form, you are responsible to return all items
- Note the appropriate check in date
- Please refer to the SML policies page for official policies regarding late fees, lost equipment and broken equipment
Return Equipment Procedures
- Return all listed equipment on the designated check in date
- Equipment will be cross referenced with the SML database and checkout form
- A returned confirmation form will be provided to you for your records
- Always return equipment to the same location it was checked out from
- SML-East or ITL-East
