University of Illinois Social Security Policy at UIC --

UIC Seminar Registration Page

You are invited to attend a UIC campus-wide meetings about the implementation of the University of Illinois policy on Social Security Numbers.

Skip to time, location, and registration.

Background

Under the terms of the U of I SSN policy, by Fall, 2002, any point-of-service operation asking for an SSN must display an approved disclosure statement as mandated by federal law. By the year 2005, the University ID Number (UIN) will replace the Social Security Number (SSN) as the primary identifier in university-wide and departmental computer systems.

Presenters

  • Ahmed Kassem, Vice Provost, CIO, and Director of the Academic Computing and Communications Center.
Ahmed is the UIC campus administrator for the SSN policy implementation.

  • Mike Corn, Associate Director, University Office for Planning & Budgeting.
Mike chaired the SSN policy committee. He is the university administrator for the SSN policy implementation.

  • A representative of the University Counsel's Office.

Topics covered

  • Laws & regulations governing use of SSN
  • University SSN policy & implementation plan
  • How UI Integrate will use SSN and UIN
  • How to decide if your process or application requires modification
  • Steps to take to modify your process or application to use UIN
  • How to write an SSN Disclosure statement
  • Resources available for conversion to UIN

And the handout from the seminar: UIC SSN Policy Implementation (PDF; 663KB).

  • Requests an SSN on a form or application from students, staff, or the public.

    • Departmental web page using SSN as a password
    • Campus Charitable Fund Drive Payroll deduction form
    • Application for non-credit 2-week program

  • Asks for an SSN over the phone to provide a service .

    Examples:
