Voice Services

 

The customer service area for Voice Services can be reached via ext. (312) 413-4475.

The Customer Service area is designed to assist University customers with their telephone needs in the following areas:

  1. Telephone Services
    1. new installation
    2. disconnects
    3. relocations
    4. programming changes
  2. Features
  3. Restriction Levels
  4. Type of Telephone Sets
  5. Repair

A Telecommunications Services Requisition and Single Line or Multi-Line Survey Sheet must accompany all telephone requests. The Telecommunications Requisition must be completed with general contact information, an authorized signature and a FOPAL account number.

All survey sheets should indicate the following information:

  1. Building
  2. Room
  3. Jack
  4. Floor Plan (if a new jack is required, mark with a red "X" to indicate the jack location)
  5. Telephone Number (if a new number is required write "New Install")
  6. Restriction
  7. Requesting to purchase a basic single line set (2500 set)
  8. User's Name
  9. UIN#
  10. NetID
