Transfer the number, purchase the equipment. In order to transfer the phone number to your personal account
and purchase the equipment please download and complete the form labeled "Personally Liable Form" as well as the "Mobile Device Purchase Form".
Once both forms have been completed please contact the ACCC by sending a note to cellphones@helpdesk.cc.uic.edu; you will
receive a response about the actual cost of the equipment. The cost of the phone to the employee will be calculated using a straight-line depreciation over four years.
For example, if the original cost of the phone was $100, and the phone is one year old, the cost to the employee is $75. If it is two years old, the cost is $50, etc..
Send the completed forms along with a check for the confirmed purchase price to:
