Advisor of the Year Nomination Form


Please use this form to nominate a student organization advisor for outstanding work and dedication. This advisor has made an impact on a student organization throughout the academic year based on his/her involvement and mentorship. In your nomination, provide descriptions for as much of the following criteria as possible.

-- Does not have to be a faculty member
-- Does have to be a UIC employee
-- Must be in a role to advise committees, groups, organizations
-- Must be an active participant
-- Encourages and facilitates development of student leaders and organization members
