STUDENT ORGANIZATION ONLINE REGISTRATION



PLEASE READ THE BELOW CAREFULLY AND COMPLETELY

Campus Programs is implementing an online Re-Registration process that will hopefully make the process easier for our students. Please keep in mind the following:
  • Your organization is not considered registered until ALL of the forms have been filled out.  These forms consist of:
    • Student Organization Registration Form (online)
    • Officer Information Form (online)
    • COF Registration Form (online)
    • Volunteer Survey (online)
    • Signature Form (hardcopy)

    EACH of these needs to be filled out in its entirety.

  • Filling out all the online forms may take a significant amount of time for some organizations.  As such, you are able to do one online form at a time and return to do the others at a later time.  After each form is completed and you click “Submit,” a confirmation email will be sent to the person completing the form.  If different members of the organization are filling out the forms it will be your responsibility to keep track of what forms have been completed.

  • You will need to be prepared with a significant amount of information to fill out the online forms.  The information you should be prepared to fill out for each form is as follows:
    • Student Organization Registration Form (this information is available to the public):
      • Official Name of Organization
      • Primary Type of Organization (a category list will be provided)
      • Purpose and Goals of Organization
      • Membership Requirements and Obligations
      • Types of Activities and Events Planned
      • Primary Contact Info for the Organization-Main Contact Person’s Name, Number, Email, the Organization Website address, Office Phone and Room # (if applicable)

    • Officer Information Form (this information is not made public):
      • Each Officer’s Title (up to 7 can be listed)
      • Each Officer’s Name
      • Each Officer’s UIN
      • Each Officer’s Primary Phone #
      • Each Officer’s Primary Email Address
      • Each Officer’s Current Address
      • Your ADVISOR’S Name, Department, Office Room # & Building, Mail Code, Phone #, and Email

    • COF Registration Form (this is your organization’s on campus financial account; this information is not made public)
      • Name of Organization
      • Purpose of Organization Account
      • Sources of Funds
      • Officer information (see b. above) for at least two officers, with an optional third officer, who will have access to your organization’s account

    • Volunteer Survey (this information will be shared in aggregate, organization specific information will not be shared)
      • General information regarding your organizations participation in, or planning of, volunteer, service, or philanthropic activities (if any)
      • Approximate number of activities planned per semester (if any)
      • Average number of participants (if any)
      • Charities or social organizations you work with on a consistent basis (if any)
      • Approximate amount of money raised for philanthropic events (if any)

    • Signature Form
      • This is the last piece that should be completed
      • All names and signatures have to match the officers (and advisor) listed on the Officer Information Form and the Chicago Organization Fund
      • There is an Anti-Hazing and Non-Discrimination Policy that the President will have to read and sign

    • Newly registered student organizations must submit a constitution along with the Signature Form. Click here for a sample constitution.

  • IF YOU MAKE ANY ERRORS, or need to change information on a form after you have hit “Submit,” you will need to open a new form, choose the “Just Updating” option (not the “Re-Registration” option) and fill out the name of the organization, the net ID of the person making the changes, and the fields you need to correct only.

  • Please keep in mind that in order to maintain your registration, and to complete all registration requirements, your organization will need to have two officers attend an SOR Workshop.  The dates, times, and locations will be announced via the UICORGS listserv at a later date.
