Advisor of the Year Award Nomination Form

Please use this form to nominate an advisor for outstanding work and dedication to improving a student organization and campus life.  The Advisor of the Year award is given to the individual whose exemplary service has not only benefitted the students with whom they work, but also the UIC campus and surrounding community.

Please provide supporting documentation (such as examples of programs, letters of recommendation, or other supportive material) not to exceed four pages. Submit this information along with a nomination form online or in paper copy to Campus Programs, 340 UIC Student Center East by Friday, March 12, 2010 at noon.
