Commitment to Diversity Award

Please use this form to nominate an organization or an event that has educated the campus community on issues related to diversity.  This includes, but is not limited to, issues pertaining to women, racial/cultural, religion, gender and sexuality.  If the program was co-sponsored by two or more organizations, all groups are eligible for the award.

Please provide supporting documentation (such as examples of programs, letters of recommendation, or other supportive material) not to exceed four pages. Submit this information along with a nomination form online or in paper copy to Campus Programs, 340 UIC Student Center East by Friday, March 12, 2010 at noon.
