Student Organization of the Year Award Nomination Form

Please use this form to nominate an organization for outstanding work and dedication to improving campus life.  This organization must have proved to be a shining example to other organizations throughout the year, based on all aspects of organization responsibility, including, but not limited to, quality of programming, active participation of membership, successful recruiting and organization management. In nominating your choice for Organization of the Year, provide descriptions for as much of the following criteria as possible. The organization must have been in good standing for this full academic year and must currently have full recognition now.
Please provide supporting documentation (such as examples of programs, letters of recommendation, or other supportive material) not to exceed four pages. Submit this information along with a nomination form online or in paper copy to Campus Programs, 340 UIC Student Center East by Friday, March 12, 2010 at noon.
