Registration categories and fees are shown in the schedule, below. All fees are in U.S. dollars. Note that discounts are available for those who register by May 15, as well as trainees and students who register for the full conference.

Your Registration Fee Includes:

  • Full conference registration allows for participation in all symposium sessions and social activities. One banquet ticket is provided with your registration. Additional/guest tickets are available for purchase.
  • Selected day registration allows for participation in symposium sessions on individual days. Registrants in this category have the option of purchasing banquet tickets for themselves and guests.

Discounted Fees:

  • Discounts are available to those whose registration forms and payments are received by May 15.
  • Additional discounts are available to trainees, including graduate (pre-doctoral) students, postdoctoral (MD or PhD) fellows, medical residents, and medical students. To qualify for trainee discounts, verification of status must be included with your registration―a letter from your program director or supervisor, department head, college or university.

Important: For Security reasons, please do not submit credit card information via E-mail or the Internet at this time. It is anticipated that online payments will be possible in the near future. This message will be removed at that time.

All payments must be made in U.S. dollars. Checks and money orders are payable to the University of Illinois and must be drawn on a U.S. bank. Faxed registrations are accepted if paid by credit card (MasterCard or Visa). Phone registration is not available.

Refunds

Cancellation and refund requests must be confirmed in writing, and all refunds are subject to a minimum $25 deduction for processing. Requests received after May 30, but on or before June 15, are refundable, less $100 (total). No refunds will be issued for requests received after June 20, 2003, or for non-attendance.
