ON-LINE REGISTRATION



FEE, DEADLINE, PAYMENTS
The registration fee is $990. A reduced rate of $890 is available to physicians-in-training (with verification of status), as well as groups of three or more registrants from a single institution whose registrations and payments are received together. The registration deadline is August 1, 2005.

Payment, in U.S. currency, may be made by credit card (MasterCard or Visa), check or money order. Checks and money orders are payable to University of Illinois. Vouchers, purchase orders, or phone registrations are not accepted. There is a $25 service charge for all returned checks.


TO REGISTER
ON-LINE REGISTRATION—We encourage you to register online, regardless of the payment method you plan to use.
We offer a secure site for online credit card purchases, but you may also opt to simply create your account online, input your registration information, and indicate a preference to submit payment separately (check, money order, or credit card information). Online registrations with accompanying payment will be confirmed immediately. Pending status will be assigned where another payment option is selected. (Note that you will not be officially registered until payment is received and pending status is removed from your account.) Once established, you may view your account and/or update profile information at any time.

CANCELLATIONS/REFUNDS
Cancellation/refund requests must be confirmed in writing to the UIC Office of Conferences and Institutes, and all refunds are subject to a minimum $50 processing fee per registrant. Requests received after August 1, but on or before August 22, will be subject to a deduction of $350 per registrant. Refund requests for fees paid at the group discount rate will be subject to additional adjustment if the remaining members of the group no longer meet the minimum-of-three requirement. No refunds will be issued for requests received on or after August 22, 2005, or for non-attendance.
