Forms
Most OSFA forms are not available for download. The OSFA’s imaging software requires very strict print specifications, so we print our own forms whenever possible to maximize the efficiency of the software.
For convenience, certain forms are available for you to download and print on your own. Please make sure that you need to complete a form before filling it out and submitting it to the OSFA, and make sure to select the appropriate year form. Print legibly on all forms to ensure that documents can be read and processed.
