What Information does GLRSN gather about you?
When you decide to register for a quality of care teleconference at GLRSN's site, you will be asked to provide your name, hospital address, e-mail address, ZIP code, telephone number, and other information. This information is required in order to provide continuing education units (CEUs) through Oakton Community College . Even if you are not requesting CEUs, this information is still collected in order for the Great Lakes Regional Stroke Network to provide information to our funder, the Centers for Disease Control and Prevention.
When you give us your e-mail address, we will provide a confirmation to the call as well as send the CEU certificate to that address. We will not use these email addresses for any other purpose. Please note that our evaluation form does not collect any personal information.
GLRSN collects personal information to provide you, the user, continuing education units (CEUs). It also helps us identify states where additional promotional efforts about the GLRSN is needed.
GLRSN will disclose your personal information to Oakton Community College to fulfill your request for continuing education units (CEUs). We do not give, rent, lend, or sell individual information to any advertisers.
