Fully complete the personal information section at the top of the page? Be sure your name is legible and that you have included your mail code, college and department, and most importantly, your UIN number.
Indicate which Umbrella Charity or individual charities and the dollar amount you are donating to each per pay period in the correct column?
Sign and date the bottom of the pledge form? If you are designating a new or changed payroll deduction you must sign the form or it will be returned to you for completion.
Ongoing Donations are made through Payroll and are deducted per "pay period". They are continuous and ongoing until cancelled.
Pledge forms should not be submitted by employees with continuous deductions unless the employee intends to change their current deduction, add new deductions, or cancel one or more deductions.
Minimum payroll deduction per pay period, per charity is $2.00.
Continuous Deductions are ongoing until cancelled in writing by the employee. The designated amount is per pay period regardless of total dollars or number of pay periods throughout a month or year. Changes in an employee's job classification only affect the number of times the deduction is applied.
Employees are responsible for notifying HR Special Programs, in writing, if there are any changes or cancellations to be made to their ongoing payroll deductions.
One Time Donations can be made by check. Make your check(s) payable to CAMPUS CHEST. The Umbrella Charity (i.e., Global Impact) and the sub charity can be identified on the memo line (i.e. 901-0250 for Heifer International). One time donations should be accompanied by a pledge form.
