Performance reviews for Faculty, Academic Professional, and Open Range Civil Service staff are a factor in determining employee compensation under University policies related to merit-based salary programs.
Performance Planning requires dialogue between manager and employee to establish clear and specific expectations about job performance. The Planning and Performance Expectations form is provided as an instrument to document the outcome of a planning/expectations discussion.
Often, managers are aware of some aspects of an employee’s performance but not others. The Continuous Improvement Employee Feedback Form may be used by employees (on a monthly, quarterly, or as needed basis) to provide job performance information to their managers in and raise awareness about work related activities and accomplishments for which a manager does not have direct oversight.
The Continuous Improvement Manager Feedback Form may be used by a manager to provide informal feedback to an employee as needed or on a monthly or quarterly basis.
The Self Review is an opportunity for employees to document perceptions of their work performance annually. Employees are encouraged to complete the Employee Self Review Form and provide specific, behavioral-based, examples of their job performance.
The Employee Performance Review Form should be completed annually as part of the annual performance review process. Managers will review and discuss information reflected in the Employee Performance Review Form during the Annual Review Meeting.
The Individual Development Plan Form is a tool to help employees set goals and develop plans for competency improvement, link employee development and their contributions to the college/administrative unit /departmental goals and seek professional development opportunities to enhance the employee’s career at UIC.
The Performance Improvement Plan form is a tool to help managers and employees resolve performance issues by developing a plan to address concerns and to document and discuss specific improvement points, required actions and timeframes for completion.
The EPR Tracking Tool (EAS authentication required) allows managers and supervisors across campus that are responsible for completing performance reviews to indicate completion of annual employee performance reviews.
