Forms at the Documents Department

The Documents Department at the 3rd floor of the UIC Main Library has a collection of reproducible tax forms for federal tax forms required by the Internal Revenue Service as well as the State of Illinois. Forms are available for both 1997 and 1998. Note that not all forms are available here; there are still some forms for which you may need to check additional sources. However, the reproducible forms at Documents provide a useful supplement to the smaller number of forms available in the 1st floor Reserve Reading Room. Because forms in the Reserve Room do not need to be photocopied, you may wish to check Reserve Room holdings first before visiting the Documents Department.
To request tax forms at the Documents Desk, you will need to bring I.D. to the desk and ask the librarian for the forms you need. The librarian will hold your I.D. and will give you reproducible pages which you will need to take to the copy machines on the 3rd floor to photocopy. Copy machines on the North side of the 3rd floor accept both change and Copy Cards; if you wish to purchase a Copy Card, you can do so at the South side of the 3rd floor in the Microforms Department.

For your convenience, many forms can also be downloaded directly from the Internet. This page provides links to forms available online from the Internal Revenue Service and the State of Illinois, as well as other U.S. states.
