How to handle damaged/missing CD-ROM in MFHD & item record

In a case when a CD-ROM(s) that accompanies a book is reported damaged or missing, by Circulation Department, make a change in the holdings record of the item as follows.

In an 867 field:

  1. change the #z to #x
  2. notate the status of the CD-ROM (either damaged or missing, as the case may be) and
  3. add the date to document when the change was made.

Do not make any changes to the item record. Circulation Department will take care of the item record. All Regional sites will also follow the same procedure.

See an example below:

	852   20   CHS #h WY100 #i M526e 2005
	867    0   #z 1 CD-ROM (4 3/4 in.)

if the CD-ROM is reported damaged or missing, make a change as follows:

	852   20   CHS #h WY100 #i M526e 2005
	867    0   #x 1 CD-ROM (4 3/4 in.); damaged; 10/14/05
