• state licensure
• graduate / degree verification
• hospital privileges, etc.
• copy of MSPE/Dean's Letter for fellowship

2. Refer to the Office of Admissions and Records - Student Records web page for the following (FEES DO APPLY). Check the web site accordingly; failure to include fees may delay the processing of your request.

• Official University Transcript
• Authentication of Diploma and Transcript
• Copy of Diploma

3. Verification of Residency Program Completion

• Residency Program verfications are completed by our Graduate Medical Education Office
