Information about Clerkship Performance Student Evaluation formsM3 Clerkship Clinical Performance Reports Processed by the COM Registrar's Office. Evaluations are entered into an electronic web application by the Hospital Site Director and submitted to the Department Program Director at UIC. Once the completed evaluations is reviewed and approved, by the Program Director, they are submitted electronically to the COM Registrar's Office for final processing. A final grade is calculated once a passing clinical grade and passing shelf exam score have been received by the COM Registrar's Office. Your clinical grade, shelf exam score and final grade, once calculated, can be accessed via the College of Medicine On-line Score and Grade Summary application. A final grade letter and a copy of the clinical performance report is sent to you via USPS. M4 Clerkship Performance Evaluation forms Processed by the COM Registrar's Office. Evaluations are received and processed daily. Final grades are entered into the College database and can be accessed immediately via the College of Medicine On-line Score and Grade Summary application. A copy of your evaluation is sent to you, via USPS. Your mailing address (MA) should be kept up-to-date with the University Web for Student / Student Self Service system. The original copy of your evaluation is given to the Office of Student Affairs to be included in your academic file.
