REGISTRATION DATES
Advance Registration opened to the College of Medicine (COM) students
for the Summer and Fall of 2009 on Tuesday, April 7, 2009. Students can register
at their designated appointment time, based on your Time Ticket, or
any time thereafter within the allotted Summer and Fall registration
times and dates. Since the COM and UIC calendars do not typically
coincide; this letter will assist you in registering in a timely
and accurate manner.

REGISTRATION REQUIREMENTS AND INFORMATION
- All UIC students including those in the COM register for courses through the campus registration system. The registration mechanism is the UIC Web for Student / Student Self Service.
- The COM requires that you register for ALL basic science courses and clinical rotations before they begin. Failure to do so will result in an COM Advising Hold being placed on your student record. This Hold can only be cleared following receipt of an approved petition from the Senior Associate Dean of Students.
- The campus requires that registration be completed by the 5th
day of the Summer semester and the 10th day of the Fall and Spring
semetser to avoid a $50 late registration fee. The last day of
Open Registration for the Fall semester is Friday, September 4th. There is an Add/Drop and Late Registration period
between August 31 and September 4 , but COM students
are strongly encouraged to register for all classes and rotation
during the Open Registration period, before classes and rotations
begin.
- After September 4th , you must submit an email or written
petition to Dean Kashima or Dean Lantz. You must include why you did not register
on time. Once your petition is approved, you must come to the
Office of the Registrar to completed required registration paperwork. A $50
late registration fee will be assessed when tuition and fees
are assessed.

COM-SPECIFIC INFORMATION
- M-1 and M-2 students: For the Fall- You are responsible for your own registration.
- M-3 and M-4 students: You are responsible for your
own registration for the Fall semster, for rotations which begin between August 24, 2009 through January 10,
2010, inclusive for the Fall.
- Please register for ALL courses and clinical rotations that apply to you.

REGISTRATION HOLDS
- If you have a Hold on your record, you will not be able to register until it is cleared. You must check the UIC Web for Student system regarding any Holds that may be placed on your record.
- Check your registration eligibility at least three times per semester, as a hold can be added at any time. If you do not address whatever issue precipitated the hold, you will not be permitted to attend classes or clerkships, nor will you be allowed to sit for examinations.
- Registration is the vehicle by which the UIC academic transcript is created. If you do cannot register because of a hold, there will be no place to post your grades, and thus, no transcript of your experience here.

SPECIAL PROGRAM INFORMATION
- Decompressed Program students should register for only those courses which follow the prescribed Decompressed Program schedule.

GENERAL INFORMATION
- Students must inform both the COM Office of Student Affairs and the University Office of Records and Registration of any name and/or address changes. Failure to do so as promptly as possible could mean that you will miss important or timely information.
