New appointments also require completion of the full packet , but may be submitted at any point in the academic year for review at the next committee meeting (see schedule for deadlines).

Forms

For faculty being appointed as or promoted to the rank of Associate Professor or Professor in the Academic (RT), Academic (CT), Research or Clinical Discipline Track, electronic submission (MS WORD file for the forms accompanied by one PDF file containing all of the scanned external letters and documents) along with hard copies of the following forms are necessary:
