Please do not use the promotion and tenure forms available online through the Provost's Office website, but rather use those available here at the COM's Faculty Affairs site as we use a customized format for sections relating to the evaluation of teaching.

New appointments also require completion of the full packet , but may be submitted at any point in the academic year for review at the next committee meeting (see schedule for deadlines).

Forms

For faculty being appointed as or promoted to the rank of Associate Professor or Professor in the Academic (RT), Academic (CT), or non-tenure Research Track, electronic submission (MS WORD file for the forms accompanied by one PDF file containing all of the scanned external letters and documents) along with hard copies of the following forms are necessary:
