General Patient Care:

We offer comprehensive general clinical services at two locations, the UIC Outpatient Care Center in Chicago and the UIC Dermatology Associates in Palos Heights. Our services at these two facilities include the following:

Pharmacy services:

A dedicated dermatological pharmacy is conveniently located within the dermatology center. Janice Lazar, PharmD, has a long-standing interest and expertise in dermatological therapy, including compounding of medications to meet the unique patient requirements. Physicians may contact Dr. Lazar at (312) 996-8675 for consultations and compounding needs. The pharmacy also offers a mail-order service for patients.

Directions via car:
From the North, take I-90/94 South, Kennedy Expressway, to I-290 West, Eisenhower Expressway.
From the South, take I-90/94 North, Dan Ryan Expressway, to I-290 West, Eisenhower Expressway.
From the West, take I-290 East, Eisenhower Expressway.
Exit the Eisenhower Expressway at the Ashland/Paulina exit. Proceed South on Ashland Avenue to Taylor Street. Turn West onto Taylor and proceed approximately two blocks to Wood Street. Turn South (left) on Wood Street for the entrance to the parking lot. The UIC Outpatient Care Center is at 1801 West Taylor Street, next to the parking garage.

Directions via CTA elevated train:
Take the 54/Cermak Blue line train to the Polk Street stop. Walk west one block on Polk Street to Wood Street. Turn left and walk south on Wood to Taylor. Cross Taylor, turn right and walk half a block to the entrance.

Directions via CTA bus:
Take the #37 Sedgwick bus or the #9 Ashland bus to the Taylor Street stop. Walk west on Taylor Street to the entrance.

From the North, take I-90/94 South, Kennedy Expressway, to I-55 South, Stevenson Expressway. Exit at Harlem Ave (exit #283), Turn South (left) onto Harlem Avenue.
From the Northwest, take I-294 South, Tri-State Expressway, to 95th street East exit. Turn South (right) onto Harlem Ave.
From the West, take I-55 North, Stevenson Expressway. Exit at Harlem Ave, turn South onto Harlem Ave.
From the South, take 159th Street to Harlem Avenue, turn North (right) onto Harlem Ave.
Proceed on Harlem Avenue to West College Drive/West IL-83 (approximately 119th block), turn West on College Drive and proceed for another 4 blocks to 7600 West College Drive. The medical building is on the right. The parking lot entrance is in front of the building.

Directions via CTA/Pace:
Take the Orange line train to the Midway stop. Transfer to #386 Pace bus to College Drive. Walk 0.6 mile West to 7600 West College Drive.
