Welcome!

The Department of Family Medicine at the University of Illinois at Chicago Medical Center provides high quality, comprehensive health care for women and men of all ages, infants through adulthood. All our family physicians are board-certified and are active members of the faculty of the UIC College of Medicine. They are dedicated to the advancement of medical knowledge through education and research in addition to superior service to their patients.

Since we are a training center for medical and nurse-practitioner students, at times you may be asked to allow a supervised student to participate in your medical care. In addition, we operate a residency training program in our Family Medicine Center. Residents are licensed physicians training to be specialists in Family Medicine. They see their own patients under faculty supervision in our offices for the three years of their graduate training. When you work with these trainees, you have the advantage of two medical minds contributing to your care as well as the satisfaction of knowing that you have added to the education of future health care professionals.

The Family Medicine Center at OCC is located on the 2nd floor on the north side of Taylor Street. You may enter the building directly from floors 3 and 6 of the Wood Street parking structure located just south of the building. If you do not park in the structure, you may enter the building at street level using the main entrance on the south side of Taylor street.

The Family Medicine Center at University Village is located on the 2nd floor on 722 West Maxwell Street. Parking is located at the corner of Maxwell Street and Union Street.

Information services are available in the main lobby and at the parking structure entrances.

Services

Appointments can be scheduled in person or by phone. When you schedule an appointment, we'll need to know some basic information:

  • Your name, current address and telephone numbers;
  • Your birth date;
  • Health insurance information;
  • Your primary care physician or nurse practitioner's name;
  • Specific information about why you need to see a clinician so we can schedule the appropriate amount of time to attend to your medical condition.

If you are calling for a health maintenance visit (for example: a pap smear or general check-up), your appointment may be 4-6 weeks away. This allows us to leave a number of daily appointment spaces open for patients who are ill and need to be seen right away.

Please be sure to bring your insurance identification card with you when you come to the Center for your appointment. You should examine your card to see if your insurance requires you to pay a "per visit co-payment" which is usually $5, $10, or $20. Co-payments must be paid at the time of your visit, and must be paid in cash, check or by credit card.

Please arrive 15 minutes prior to your scheduled appointment time to complete the check-in process. If you are more than 15 minutes late for your appointment, you may be asked to reschedule your appointment to another day.

Urgent Care

If you need to speak with a doctor at any time, day or night, call the Center at (312) 996-2901. During office hours press option #4 to speak to our triage nurse. We try to always keep a number of URGENT CARE appointments open for patients who have a medical condition that must be cared for that day. The nurse will discuss your urgent problem with you immediately. The nurse can then determine if your condition requires immediate attention, and if so, schedule an appointment that same day or the very next day.

After business hours, your emergency calls are taken by an answering service that will notify the family physician on-call. He or she will return your call as soon as possible.

Cancellation of Appointments

If you find you are unable to keep your appointment, please let us know as soon as possible. We can then reassign your appointed time and make new arrangements for you. Patients who make appointments and fail to show up prevent other patients from accessing needed health care services.

Prescription Refills

When you are running low on prescription drugs you take regularly (e.g. birth control pills, medications for chronic diseases), please first check your drug container. If the label says that you still have a number of refills left on the prescription, call your pharmacy directly; you do not need another prescription from your physician. If the label says you have "0" refills left, or "call doctor for refill" you should make plans well in advance of running out of your medication to get a refill.

1) On the day of an appointment, let your primary care provider know you will need refills before you leave the Center.

2) If you need a refill and do not have an appointment scheduled, call our office and leave the message that you need a prescription refill.

If you are unsure of what to do, ask for the triage nurse and she will be happy to assist you.

Students

All students currently enrolled in classes at the University of Illinois at Chicago are eligible to receive some outpatient services at the Family Medicine Center. (Click here to visit the Student Health page). For those students who also purchase coverage through CampusCare, the family physicians in our Center will provide comprehensive primary care services for you as described in your member booklet, as well as management of any necessary specialty care.

Students who do not have CampusCare should consult their health insurance policies to determine how to best manage their larger health care needs. All students must present their student ID cards to initiate health care services at the Family Medicine Center. CampusCare members must also present their MegaLife ID cards, and all other students must present cards indicating their other insurance coverage.

Please note: if you are under 18 years old, you must have a consent form signed by your parents or guardians authorizing your treatment. Except for issues related to birth control, pregnancy and sexually transmitted diseases, Family Medicine cannot treat anyone under 18 without this consent
