New: Brain Aneurysm Support Group For New Patients: Please take time to read the information below before your scheduled appointment with one of our physicians. Please be aware our clinic has moved to the Outpatient Care Clinic (OCC) 4E as of July 1, 2007. When can I get an appointment? We schedule new patients within 1 week of a request. Emergency cases will be considered as an exception and will be handled promptly. What do I need to bring to the appointment?
What can I expect during the visit? You can expect to be seen in a professional and comfortable environment and to receive prompt and pleasant service. You will be treated with the utmost respect, understanding and attention by knowledgeable, caring experts. Upon review of studies, examination and history taking, your physician may order further studies (MRI, CT scan, angiogram) or recommend possible surgical treatment. How soon can I be scheduled for surgery? This is upon the discretion of your treating physician or nurse practitioner. You will need to discuss this during your appointment. Who may I call if I have questions regarding my visit or scheduled surgery? During business hours please call 312-355-0510 and ask to speak with one of our nurses who will contact your physician if the need arises. Please keep in mind that University of Illinois at Chicago Medical Center is an academic institution. This means that you will encounter ancillary personnel during the course of your treatment. This includes clinical pharmacists, nurse practitioners and surgical residents. Please feel free to ask these individuals for any assistance during your treatment. How can I obtain medical records? The department of Neurosurgery does not dispense medical records. All medical records are the property of the University of Illinois Medical Center at Chicago. For requests of records, please contact the appropriate department below. A written authorization is required. Please allow 1-2 weeks for the handling of your request.
