The OCAIRS manual includes "OCAIRS Rating Criteria", "OCAIRS Rating Scales and Notes Summary Form", and "OCAIRS Data Summary Form1 & 2". In each of three interview formats (Mental Health, Forensic Mental Health, and Physical Disabilities), there are four interview formats including "Questions", "Questions, Rating Scales and Notes Form", "Question and Rating Scales", and "Key Words Form". Three case studies are used to demonstrate analysis and planning using the OCAIRS. All forms are reproducible. This assessment is discussed and illustrated with a case in chapter 17 of The Model Of Human Occupation: Theory and Application (Kielhofner, 2008).
