The AMPS is an observational assessment based on MOHO that is used to measure the quality of a person's performance of domestic (instrumental) or basic (personal) activities of daily living (ADL). The quality of the person's ADL performance is assessed by rating the effort, efficiency, safety, and independence of 16 ADL motor and 20 ADL process skill items.
The AOF-CV is a semi-structured, self-report instrument designed to yield qualitative and quantitative information about how a person is functioning relative to key components of MOHO. It can be administered as either an interview or a written self-report. It is appropriate for use with adolescents and adults across many populations, but in particular with the elderly in in-patient settings, individuals with schizophrenia, and persons with alcohol dependence. Therapists with thorough knowledge of the Model of Human Occupation and good interviewing skills will likely make best use of the yield for clinical reasoning. The Modified Interest Checklist gathers information on a client's strength of interest and engagement in 68 activities in the past, currently, and in the future. The main focus is on leisure interests that influence activity choices. The checklist can be used by adolescents or adults. Download

The UK Modified Interest Checklist gathers information on a client's strength of interest and engagement in 74 activities in the past, present and future. The NIH Activity Record documents an individual's participation in occupations by half- hour intervals throughout the day. Users categorize the activity and record pain, fatigue, level of difficulty, perception of competence, value, and enjoyment for each activity. The questionnaire can be used by adolescents or adults. The Occupational Questionnaire documents an individual's participation in occupations by half-hour intervals throughout the day. This is a shortened version of the NIH Activity record. Users classify the activity as work, play, or leisure, and record their perception of competence, value, and enjoyment for each activity. The questionnaire can be used by adolescents or adults. by clinicians in the UK and is designed to be completed in discussion with
the wider multi disciplinary team at the point of referral. It has proved
to be a useful a starting point for screening referrals to occupational
therapy and introducing the framework of the Model of Human Occupation,
paving the way for more in-depth assessment of occupational function.

Clients answer questions about their interest and participation in a variety of play and leisure activities. Three activity scales, the Kid Play Profile, The Preteen Play Profile, and the Adolescent Leisure Interest Profile, make this self-report/interview profile appropriate for clients ages 6-21. The Residential Environment Impact Survey (REIS) is a non-standardized, semi-structured assessment and consulting instrument designed to examine the environmental impact of community residential facilities on the residents. Using four different methods of data collection, the REIS assesses how well the home is meeting the needs of the residents as a whole. Findings from the REIS can be used to guide intervention aimed at modifying the residential environment. While designed with adults with mild to moderate intellectual disabilities in mind, the REIS can be adapted to other residential settings. The Role Checklist was designed to obtain information on client's perceptions of their participation in 10 occupational roles throughout their life.
