On our website we have donor packet request forms and our contact info that you can feel free to give out to other potential donors or healthcare professionals that treat MS.

In addition, the UIC MS Tissue Repository would be more than happy to mail you donor registration packets, brochures, outreach letters and other materials to furnish you with supplies that can be freely distributed to MS support groups, physician's offices and other locales as necessary.
