Step 4 - Application for Appointment; Letters of Recommendation
If you did not include a Personal Statement with your Notice of Intent, send a one- to three-page typewritten Personal Statement describing your personal and academic achievements, discussing your career goals and indicating the basis of your desire to continue your academic career in the Graduate Program in Neuroscience at the University of Illinois at Chicago to the address below. You can also send your statement as an email attachment to the Program Coordinator. Adobe Acrobat "pdf" format is preferred - Word documents will be accepted.
Download the Request for a Letter of Recommendation (pdf format). Carefully read and complete the top of the form before submitting the form to those individuals whom you are asking to provide a recommendation. Ask the provider to include this form with their letter.
The Personal Statement (if not sent electronically) and Letters of Recommendation must be addressed to:

Applications are accepted for the Fall semester only. The program's preferred deadline is December 15th. All applications must be received by the Office of Admissions and Records (OAR) no later than February 1 for consideration for the following academic year. All other supplemental material must be received by OAR and the Program before any decisions on admissions can be completed.
