PAPER APPLICANTS CHECKLIST

ONLINE APPLICANTS CHECKLIST:

Please send the following documents to the addresses listed below:

• UIC Application
• Honors College/GPPA Programs Supplemental Application
• ACT/SAT Test Scores – Please have these scores sent to UIC. 
  (We will accept official ACT or SAT scores as posted on your official high school transcript.  If your high school does not report these test scores, you must have them sent directly from the testing service.)
• Official H.S. Transcripts
  • For GPPA Applicants, please send one copy to:

• Application Fee - $40 made payable to the University of Illinois at Chicago
• Letters of Recommendation – 3 letters are required.
  • GPPA Programs Applicants – One set of three letters of recommendation sent to the Special Projects Unit in Admissions.  (Please print the downloadable form here.)
  • Honors College Only Applicants – One set of three letters of recommendation sent directly to the Honors College.  (Please print the downloadable form here.)

PAPER APPLICANTS CHECKLIST:
In order to apply for the Honors College or the GPPA programs using a paper application, please note the following information:

GPPA APPLICANTS:  The GPPA paper packet contains an application for UIC admission, and a GPPA Supplemental application which serves as an application for Honors College admission, and GPPA admission.  (For those students applying to programs for which the Honors College is optional (see above), if you include the Honors College Statement you will be considered for the Honors College.)

• GPPA Application packet containing UIC Undergraduate Freshman Application and GPPA Supplemental Application
• GPPA Personal Statement
• Honors College Statement (if required)
• ACT/SAT Test Scores – Please have these scores sent to UIC. 
  (We will accept official ACT or SAT scores as posted on your official high school transcript.  If your high school does not report these test scores, you must have them sent directly from the testing service.)

HONORS COLLEGE APPLICANTS:  The Honors College paper application can be submitted directly to the Honors College after you have submitted a UIC Undergraduate Admissions application. Please include the following:

• Honors College Application
• Honors College Statements (2 required –see application for writing prompts.)
• Official H.S. Transcripts
